Live at home with lots of support

Choosing home care services can be overwhelming, but our approachable staff can help you get started.

Your first step to aged care is registering with My Aged Care on 1800 200 422 or www.myagedcare.gov.au.

Once allocated a Support at Home program, contact Barossa Village to be assigned a care partner who will work with you to deliver tailored home care services and support to meet your goals.

Eligibility

You may be eligible for Australian Government-funded aged care services if you’re an older person (usually 65+ or 50+ for Aboriginal and Torres Strait Islander people) and finding it more challenging to do the things you used to do.

You will need to arrange an assessment of your care needs through My Aged Care to determine whether you are eligible.

Types of Care

Commonwealth Home Support Program (CHSP)

The aim of the CHSP is to help older people live as independently as possible at home with a focus on working with you, rather than doing for you, to help you achieve your goals. CHSP can be actioned within four to six weeks from assessment.

All CHSP services are accessed through My Aged Care.

Enquiries can be made on 1800 200 422 or www.myagedcare.gov.au, however, we recommend that you first contact our Care Enquiry Team on (08) 8562 0300.

Support at Home

All Support at Home is accessed through My Aged Care. Enquiries can be made by calling 1800 200 422 or visiting www.myagedcare.gov.au.

Support at Home offers a wide range of support so you can continue to live well at home. Based on your aged care assessment and eligibility, you will have access to an approved list of services. You will be supported to choose a mix of services you can access within your budget.

Under the Support at Home program, you can request to change the mix of services you access from the approved list at any time with your provider, who will support you to make sure the services you receive meet your needs.

This includes support for:

  • Clinical care – such as nursing care, occupational therapy and continence care.
  • Independence – such as help with getting dressed, taking medications, transport or respite care.
  • Everyday living – such as cleaning, gardening, shopping or meal preparation.
